The work presents a historical perspective of what happened in health care in the 1990 and forecasts to come changes. The author identifies five stages of managed care market penetration, directions in ambulatory care, and the guidance regulations that originated during this period. Many of the changes the author forecasts have begun to meet the eye including consumer-driven care, improved communication between caregivers and consumer increasing numbers of ambulatory care center and increasing use of e-health technologies. The author forecasts a shift from risk avoidance to risk management and from illness intervention to health promotion, which has been a health care mantra since the 1980 He believes that the reason major changes have not occurr in health care is the lack of leadership and research. The author provides insight into the sixth stage of managed care, which includes universal coverage and shared risks with provider incentives. He says that managed care must change--patients and profits cannot coexist. The work is easy to read. Each strategy is clearly identified and succinctly described.
