BEHAVIORAL OBJECTIVES

After reading and studying the article forward triple arthrodesis, the nurse will be able to

(1) discuss bottom anatomy and the etiology of twelve inches deformities requiring triple arthrodesis;

(2) describe patient evaluation arrangements benefits, and complications of triple arthrodesis;

(3) discuss anesthesia management of patients undergoing triple arthrodesis;

(4) describe the surgical constituents of triple arthrodesis;

(5) discuss perioperative care for patients undergoing triple arthrodesis; and

(6) describe perioperative nurses' parts when caring for patients undergoing triple arthrodesis.

Arthrodesis is defined as the surgical fusion of a joint. Triple arthrodesis involves the fusion of the calcaneocuboid, talonavicular, and talocalcaneal (ie, subtalar) joints in the hindfoot with bone graft material and internal fixation (eg cannulated hunkss bone staples). The first published description of triple arthrodesis was in 1923 and it has been a universal orthopedic procedure for more than 50 years.(1) Triple arthrodesis originally was indicated for stabilization of feet paralyzed by way of polio and correction of residual clubfoot deformities in patients with myelomeningoceles.(2) Today, orthopedic surgeon perform triple arthrodesis practices for correction of marked varus or valgus deformities from residual clubfoot cerebral palsy, and Charcot's joint disease (de neuropathic arthropathy of the hindfoot); chronic acquired flatfoot from end-stage posterior tibial tendon ruptures; and arthritic conditions of the hindfoot.

Despite advances in surgical techniques and expanded indications for triple arthrodesis, the overall objectives of surgical treatments remain unchanged. They include correction of significant lower part deformities, prevention of foot instabilities, pain relief, and improvement in lower extremity function so patients can wear shoe Contraindications for triple arthrodesis include instances in which more appropriate or les extensive surgical proceedings would suffice. Alternative surgical conducts may include tendon transfers for correction of hindfoot mobility deformities or isolated joint arthrodesis of Chopart's (ie, transverse tarsal) joint, depending forward the specific joints and deformities involved.

The twelve inches is divided into the hindfoot, consisting of the Achilles tendon and the talus and calcaneus bones; the midfoot, consisting of the navicular, cuboid, and three cuneiform bones; and the forefoot, consisting of the five metatarsals and corresponding phalanges (Figure 1)(3) The talus and calcaneus are located forward the posterior aspect of the lower part The talus consists of a corpse neck, and head and articulates with the calcaneus and navicular. It is the no other than foot bone that articulates with the fibula and tibia malleoli, and it consists of posterior, lateral, and medial facets. During walking, the talus initially bears the entire weight of the carcass as the person plants his or her lower part Approximately half the body's weight then is transmitted to the calcaneus, the largest and strongest bottom bone. The calcaneus forms the heel and gives support to the talus between the sides of three facets that articulate with the talus.(4)

The lower part bones are held together through ligaments and tendons that help form a two-way arch connected view consisting of longitudinal (de, lengthwise) and lateral (de transverse) arches. These arches enable the lower extremity to support the weight of the human dead body and provide leverage during walking. The arches of the lower extremity are not rigid; they yield as weight is applied and spring back when weight is lifted. stout fibrous structures normally hold the paw bones firmly in their arched positions, on the contrary frequently these supporting structures weaken and issue in foot deformities (eg, flatfoot).(5)

The talocalcaneal joint has three articulations between the anterior, middle, and posterior facets of the calcaneus and talus. These articulations function as a single-unit, multiaxial joint that allows the bottom 40 degrees of supination and proration. Chopart's joint consists of articulations between the calcaneocuboid and the talonavicular joints. The midfoot is unbolted when the hindfoot is valgus; therefore, surgeon slightly prorate the calcaneus when they fuse the talocalcaneal joint.(6)
