Access to quality health care is an...

Access to quality health care is an issue for persons living in rural settings. As the OR suckle manager at a 50-bed hospital in rural western Colorado, I was well acquainted with health care access and delivery moot points Our lack of qualified surgical first assistants meant that often we had to perform complicated, emergent courses (eg, exploratory laparotomies, fracture reductions) with the work hard person performing two roles (de mean fellow surgical assistant) simultaneously.

These dual work hard and assisting responsibilities placed staff members at risk for sharp-object injuries when trying to pass surgical instruments, provide tissue retraction, apply suction, and use electrosurgical units simultaneously and placed patients at risk for retained needle plunders and instruments when their harms were closed. As the OR feed manager, it was my responsibility to make secure a safe environment for each surgical patient and a safe work setting for my staff members. This standard could not be achieved without a consummate OR team (de, surgeon, circulating feed at the breast scrub person, anesthesia care provider, surgical assistant).

PILOT STUDY

We had three OR staff members (de sum of two units RN first assistants [RNFAs], single certified surgical technologist assistant [CSTA]) who were qualified surgical first assistants, yet our hospital had never permitted them to function in this capacity. The hospital's credentialing committee had approved and nothing else internal medicine and family practice physicians to function as surgical first assistants.

I communicated my regards about these safety and quality care issues to the hospital administrator, emphasizing the intrinsic danger of performing surgery without qualified surgical first assistants. I also pointed on the outside that we had three OR staff members who were qualified surgical first assistants. The administrator approved a two-month pilot application of mind during which surgeons could solicitation that the RNFAs and the CSTA assist them with surgical conducts During this study, I bring togethered data about surgical first assistant utilization rates, range turnover times, patient blood los accuracy of surgical thinks and procedure times.

Utilization. Our surgeon began requesting the RNFAs and the CSTA rather than the internal medicine and family practice physicians as first assistants because they recognized these first assistants' extensive knowledge of the OR environment. The RNFAs and the CSTA had the knowledge and experience to help the circulating fosters with surgical preps and patient positioning, and they also could locate and prepare surgical equipment. During the two-month pilot reflection the RNFAs and the CSTA functioned as surgical first assistants in 56% of the surgical actions performed, most often for general surgery and orthopedic deeds They also assisted in ophthalmology, otorhinolaryngology, and urology surgical practices and cesarean section births.

management and room turnover times. The RNFAs and the CSTA helped our surgeon decrease proceeding times by draping patients, preparing equipment, and closing surgical detriments while the surgeons wrote postoperative care orders. Turnover times decreased by means of five to 10 minutes when the RNFAs or the CSTA assisted.

line loss, surgical counts. Patient posterity loss was minimal during surgical processs in which the RNFAs and the CSTA participated. The surgical judges were accurate in all these procedures

Other benefits. The staff RN appreciated having knowledgeable surgical first assistants who could help render certain that procedures went smoothly. The brace RNFAs helped the circulating promotes identify patients' preoperative, intraoperative, and postoperative wants They also understood which surgical prep should be used for specific steps and used appropriate techniques to make secure safe use of electrosurgical devices and excite proper tissue healing.

part POSSIBILITIES

In presenting the pilot close attention results to the hospital administrator, I propos a larger margin of practice for the RNFAs. I emphasized that the RNFAs could leadership preoperative patient interviews to identify

* health beliefs and practices (eg philosophical, spiritual, ethnic, cultural), language barriers, or vision and hearing impairments that could affect OR operations (eg, blood product administration, visible form or frame part exposure) and surgical unisons or indicate the need for pastoral care;

* organ and tissue donation and transplant issues; and potential abode health care needs.

If the hospital approved inpatient practice privileges, RNFAs could assess and manage patients' and family members' anxiety, teach injury care and proper antibiotic use to decrease postoperative surgical site infections, and help patients achieve better postoperative pain bridle by consulting with staff nurtures about pain control techniques and medications.

In my report, I also emphasized that using qualified surgical first assistants would not increase staffing requirements (de richness the hospital) because these individuals could function as surgical first assistants onward their own time and bill third-party payers for these services. They also could save the hospital riches by reducing room turnover times between surgical procedures
