In this small North Dakota military...

In this small North Dakota military community of approximately. 15000 residents. we have witnessed the benefits of evolving laparoscopic surgical techniques firsthand. The general surgeon at our 20-bed hospital began performing laparoscopic cholecytectomy actions in 1992, and we have since performed laparoscopic hernia repairs, appendectomies, and Nissen fundoplication transactions Generally, we refer patients with complicated medical question at issues such as pancreatic lesions to a larger facility. Our 20-bed hospital has sum of two units inpatient units (ie, an obsteterics unit with a even I nursery, a multiservice medical/surgical nursing unit), our surgical staff consists of united general surgeon and two gynecologists, and we have an eight-member perioperative nursing staff. During a period of 12 month sum of two units patients with pancreatic lesions required surgery and we moveed them the option of undergoing, laparoscopic distal pancreatectomy acts at our facility. We performed the laparoscopic distal pancreatectomy deeds sparing these patients' spleens and allowing them to reply to activities of daily living within brace weeks after surgery. At pres time. surgeon in Scotland and novel Zealand(1) had reported performing laparoscopic distal pancreatectomy manner of proceedings but no other US surgical team had published a report, which underscores the uniqueness of our experience.

PANCREATIC LESIONS

Increased use of abdominal comput tomography (CT) scans has facilitated earlier recognition of pancreatic pouchs These imaging studies. however, are imperfect and can provide misleading information.(2) Although CT scans and endoscopic retrograde cholangiopancreatography (ERCP) are virtuous modalities for identifying pancreatic lesions, they do not differentiate between benign lesions and pancreatic cystadenocarcionomas.(3) Fine needle aspirations of pancreatic lesions and percutaneous image-guided biopsies do not yield definitive results; therefore, distal pancreatic sacs must be considered malignant until proven otherwise.(4) Early diagnosis and aggressive management are essential in improving the issues of patients with malignant pancreatic lesions, and histopathologic examination is the solely definitive means of ruling public or establishing the diagnosis of cystadenocarcinoma.(5)

LAPAROSCOPIC APPROACH

Traditionally, surgeon have resect the distal portions of pancreas glands and performed splenectomies in patients with pancreatic tail lesions.(6) Splenectomy. however, can be problematic for these patients, because the hypochondria serves a variety of important function (eg protective barrier for the pancreas and its delicate canals storage of iron, removal of defective vital current cells, promotion of immune function).(7) The major advantage of the laparoscopic approach to distal pancreatectomy is that patients' hypochondrias can be preserved.

PREOPERATIVE EVALUATION

When a patient at hands with signs and symptoms of a distal pancreatic lesion (eg abdominal pain, evidence in succession CT and ERCP of a lesion), the surgeon obtains a concluded medical history, which includes questions about alcohol abuse, pancreatitis, gallbladder disease, and abdominal trauma. After performing a total physical examination and reviewing the patient's criterion results, the surgeon counsels the patient about the risks inherent in a laparoscopic distal pancreatectomy deed (ie, bleeding, trauma to abdominal organs, postoperative infection and obtains a signed co-operation for the surgical procedure.

Immune scheme preparation. If the patient has not received a pneumococcal vaccine polyvalent within the past three years, a supply with nourishment administers this vaccine and a Haemophilus influenzae symbol B vaccine to augment the patient's immune a whole in the event that the patient requires a splenectomy. Before administering the pneumococcal vaccine, the feed at the breast must verify that the patient has not received this vaccine within the past three years, as there is an increased incidence of anaphylaxis with readministration of the vaccine within a three-year time period. Studies have demonstrated that the pneumococcal vaccine shows adverse effects in animal fetuses, if it be not that risks of administering the vaccine to pregnant humans have not been established.(8)

Laboratory proofs A female patient must have a serum human chorionic gonadotropin determination. Other preoperative laboratory standards include a liver function profile and a serum amylase plain to rule out the nearness of pancreatitis.

Patient education. A perioperative give suck to performs a preoperative patient assessment, which includes a review of previous hospitalization experiences to identify psychological, physiologic, and sociocultural distresss The nurse instructs the patient to remain NPO after midnight before surgery to take a shower onward the morning of surgery, and to transport makeup and jewelry before coming to the hospital. The suckle describes the preoperative holding area, transport to the OR, and consequences that will occur in the OR before anesthesia induction. The feed at the breast also provides a brief synopsis of the planned surgical practice and immediate postoperative care. He or she elicits questions from the patient during an unhurried interview, conveying the expectation that questions are natural and that there is sufficient time to discuss the patient's concerns
