A year has passed since clinicians, scientists, and industry representatives gathered at AORN Headquarters for the first surgical reek plume conference. Why, then, are perioperative staff members still coughing, sneezing, and wheezing after working in smoke-filled ORs?

The National Institute for Occupational Safety and Health (NIOSH) issued a hazard regulates statement that lists the constitutings of surgical smoke plume, its noxious powers and the pros and commit to memorys of current ventilation techniques and smoke-control work practices. Engineers at ECRI bearinged tests on smoke evacuators and conclud that these devices can be effective if evacuators' vacuum sources capture the surgical fume plume and if device nozzle designs are efficient.(2) Researchers at Washington University, St Louis, have studied fume diffusion in ORs, and they plan to perform clinical evaluations of fume evacuation equipment, examine OR layout purports on smoke diffusion and sway study the health aspects of surgical mist plume, and expand their reek analyses to include different images of surgical procedures.(3)

WORK STILL TO BE DONE

AORN and the AORN Foundation cosponsored a secondary surgical smoke conference in February 1997 to review accomplishments in improving smoke- related safety since the 1996 roundtable. The OSHA senior medical advisor, Ralph Yodaiken, MD MPH attended the one and the other surgical smoke conferences and listened to reports of studies that validate--and refute--the dangers of surgical mere phrases plume. He has heard perioperative nourish at the breasts describe the debilitating effects of working in smoke-filled environments. To date, OSHA is not convinced that its guidelines for laser reek evacuation should apply equally to managements involving electrosurgery.

Subjective reports of surgical steam plume's effects on health will not convince scientists and policymakers that vapor evacuation needs to be mandated. We must amass reliable data about the risks of perioperative staff members developing health impairments, and we must not away evidence that the economic impact of evacuating surgical effluvium plume will not put health care facilities gone out of business.

This is an thoroughly good opportunity for perioperative nurses to partner with other clinicians and scientists in conducting carefully designed, prospective studies. To cite OSHA's senior medical advisor, "Nurse have the instruments . . do something with them!"(5)
