Total shoulder arthroplasty (TSA) has made significant advances in the past decades including improvements in surgical techniques, prosthetic designs, and operative protocols. Total shoulder arthroplasty is a complicate surgical procedure. With patient cooperation and aggressive rehabilitation, however, a TSA usually is successful

HISTORICAL INFORMATION

In 1893 a French surgeon attempted a TSA by the agency of implanting a platinum and rubber prosthesis in the shoulder of a 37-year-old baker.(1) The prosthesis functioned for sum of two units years before it was remov suitable to infection. Early surgical techniques for resection of the humerus and reattachment of the rotator box to the humerus provided pain relief, still the joint was less than operational.(2)

In the 1950 orthopedic surgeon began performing hemiarthroplasty processs on the glenohumeral joint at replacing the humeral head with a constrained prosthesis (ie, the glenoid and humeral component parts were fixed together). These prosthetics--cemented and noncemented--were the treatment of choice for 20 years. Then in 1970 surgeon began performing TSA conducts with a redesigned humeral head to fit the glenoid component



In 1973 they switched to implanting an unconstrained prothesis that had an ultrahigh molecular weight polyethylene connected glenoid component. This prosthesis was approved for use in TSA practices in 1983 and is used as a common thing [i]or[/i] matter today.(3)

SHOULDER ANATOMY

The shoulder (Figure 1) is a shallow ball-and-socket joint where the humerus joins the scapula. The socket is glenoid (ie, resembling a shallow cavity or socket) in shape. The glenoid cavity, therefore, is too shallow to provide bone stability, level though it has a fibrocartilaginous rim (ie, the labrum glenoidale). As the humeral head has three times the surface area of the glenoid cavity, the rotator box muscles provide the shoulder's stability. The rotator slap consists of the subscapular, supraspinous, infraspinous, and the tere minor muscles. These four muscles and their tendons stabilize the humeral head against the glenoid cavity, which allows the deltoid muscle to abduct, adduct, flex and stretch out the shoulder without impingement. The deltoid muscle also elevates the humeral head as in the motion of shrugging.(4)

PATENT SELECTION

Surgeon perform 90% of all TSA conducts on patients with osteoarthritis, rheumatoid arthritis, or traumatic arthritis. Total shoulder arthroplasty performances also are indicated for four--part humeral fractures and fracture/dislocations and head split fractures.(5) The primary drift of TSA procedures, however, is to bring into patients' intractable pain.(6) Surgery is contraindicated for patients with fresh or active infections or patients who present to view the inability--psychological or physiologic--to participate in postoperative rehabilitation.(7)

PREOPERATIVE PATIENT CARE

The surgeon provides the patient with basic information about the surgical process (eg, location and length of incision, use of general anesthesia), count uponed benefits, and potential risks and complications. * reviews the patient's medical chart for a history and physical examination, existing laboratory test results, and radiologic reports.
