The following make acceptableed practices were developed by the AORN make acceptableed Practices Committee and have been approved by way of the AORN Board of Directors. They were readyed as proposed recommended practices in April 1997 for observation by AORN members and others. They are effective Jan 1 1998

These attract favor toed practices are intended as achievable recommendations representing what is believed to be an optimal plain of nursing practice. Policies and actions will reflect variations in practice settings and/or clinical situation that determine the order to which the recommend practices can be fulfilled.

AORN recognizes the numerous adumbrations of settings in which perioperative feeds practice. These recommended practices are intended as guidelines adaptable to various practice settings. These practice settings include traditional ORs, ambulatory surgery units, physicians offices, cardiac catheterization suites, endoscopy suites, radiology departments, and all other areas where operative and other invasive managements may be performed.

commited PRACTICE I

Patients receiving local anesthesia during a surgical proceeding should be assessed preoperatively and continually observ during the management by an RN.

Interpretive statement 1:

The selection of patients who are to be in the way that managed should be determined at established criteria developed through an interdisciplinary collaboration of health care professionals.

Discussion:

There are many healthy patients who have minor surgical courses that require only small doses of local anesthetic medications. These patients are at reasonable risk and require minimal observation and intervention.

Interpretive statement 2:

The decision to monitor, the parameters monitored, and the common occurrence of observation should be tailored to the patient, the surgical transaction and the medications used.

Rationale:

Not all patients are candidates for local anesthesia. This emblem of anesthetic is not practical for all models of surgical procedures.(1) Highly nervous, apprehensive, and excitable patients or patients who are unable to cooperate because of their mental state or age may not be worthy candidates for local anesthesia.(2) Each patient has a variety of unique physical characteristics that can influence his or her reply to medications. Considerations include the patient's weight, age, medication tolerance, and the port of disease.(3)

Discussion:

In the preoperative phase, the RN should assess the patient's history and physical examination findings, laboratory ends and other diagnostic test terminates if indicated. The preoperative assessment should include, however not be limited to determining

* the patient's allergies to medications, tape, or prep solutions;

* the patient's age and emotional status; and

* whether the surgical site is amenable to local anesthesia.

Interpretive statement 3:

When indicated, the RN should monitor the patient's heart rate and regularity, respiratory rate, and mental status quite through the surgical procedure. The RN should also monitor the dose, passage and time of administration for all local anesthetic medications given to the patient during the surgical process Additional monitoring parameters based upon the patient's condition may include vital fluid pressure, skin condition, and oxygen saturation. Any changes in the physiologic or mental status of the patient should be reported to the physician immediately.

Rationale:

Local anesthetic medications may cause hypersensitivity reactions; cardiovascular, respiratory, and central nervous order depression; and toxicity.(4) A hypotensive reaction may happen as a toxic response to local anesthetic medications. A patient's children pressure may fall gradually or abruptly, and the heart rate may change suddenly(5) Although a patient's sensation is absent, injury to the anesthetized surgical site can occur(6) Patients undergoing a surgical manner of proceeding with local anesthesia may experience significant intraoperative bleeding.(7) They may be at risk for hypothermia from unexcited ambient temperatures, the use of unwarmed IV and irrigation fluids, or defered surgical time.(8)

Interpretive statement 4:

The RN should be knowledgeable about medication administration and be able to recognize normal and adverse medication reactions

Rationale:
