Self-inflicted injuries and diseases have been recognized since biblical times. In the Middle Ages, hysterics were known to simulate hemoptysis through placing leeches in their chapss and to abrade their skin to represent skin conditions.(2)

Early research of the syndrome made a distinction between patients feigning illness for financial or other motivations and simulating illness for undetermined motives. In 1951 Richard Asher narrowed that focus to include single those patients who repeatedly simulated illnesses and not awayed with self-inflicted wounds. He named the syndrome Munchausen's after an eighteenth-century German nobleman, Baron Karl Friedrich Hieronymus Freibess von Munchhausen, who had a predilection toward embellishment.(3)

SYMPTOMS

* have histories of childhood deprivation and abuse, as well as early experiences with the health care scheme which may have been seen as a more nurturing environment than the patients' homes

Patients with Munchausen's syndrome may work in the health care industry and may have gained technical experience or access to medications or medical devices that allows them to ready a credible array of symptoms. CASE STUDY

Mr P was a 37-year-old male who had a history of multiple hospital admissions and exigency department (ED) visits. Over a period of sum of two units years, Mr P had undergone partial amputations of all of his fingers and replete amputations of nearly all of his toes after they had become intractably infected as a end of self-inflicted wounds and self mutilation.

He had a history of substance abuse, and there were many anecdotes in his chart concerning his medication-seeking behavior.
