My sister-in-law will drive you cra...

My sister-in-law will drive you crazy! My nephew is coming in tomorrow for a T and A; you would think he had a tumor!"

When my promote colleague and friend of many years addressed me with this plea for understanding, I realized for what reason easy it is even for an experienced pediatric supply with nourishment to lose sight of the dynamics of "ordinary" child care. Her experience is not unique. I frequently have heard nurses characterize parents of children scheduled for tonsillectomy and adenoidectomy (T and A) actions as being frantic and overwhelmed. Indeed, when I first started working as the surgical liaison promote at Children's Hospital in Boston 14 years ago, my earliest lecturing was that the level of anxiety demonstrated by the agency of a parent or family member has little to do with the emblem of procedure listed on the OR schedule.

a certain quantity of mornings I would arrive at the liaison office door to find a wide-eyed parent already waiting for me The parent would ask, "How is Mary? They told me you would hold fast me informed!" When I asked, "What is Mary having done?" I used to calculate upon the answer would be cardiac surgery or a major lifesaving performance I always was surprised when the replication was, "She's having her tonsils taken out!" I began to bewilderment why parents of children undergoing other patterns of surgery were not asking the same question about their children. with what intent did this particular diagnosis and manner of proceeding generate such a high flat of stress for parents and family members? I started making a fist of questions and stres factors that might be influencing parents' reactions. These included the following.

* Had either of the parents had a tonsillectomy as a child?

* If in the way that what had that experience been like?

* Had preoperative teaching for this child's surgery been adequate?

* What issues in this child's life l to the decision for surgery? I spoke with each of the attending otorhinolargyngology (ORL) surgeon and the ORL cherish clinician. I also spoke with psychology department staff members and anesthesia care providers, as well as admitting preoperative foments and inpatient nurses. Everyone acknowledged that he or she had noticed the anxiety even in some parents, but there was no single reply that explained my observations. I also reviewed articles in professional journals, further they gave me little insight into the parents' responses

CAUSES OF STRESS

through time, and after much musing and reflection, I began to descry a pattern. One reason for the high flush of anxiety is that according to the time a child arrives at the hospital for a T and A. he or she has experienced anywhere from four to as many as 10 to 15 throat infections requiring treatment with antibiotics. Sometimes an antibiotic has been ineffective, and after seven to 10 days of therapy, a fresh medication has been added. The child may have had an untoward reaction to this antibiotic therapy (ie, vomiting, gastrointestinal cramping, rash).

Nutrition point to be solved or settleds These recurrent infections may have l to nutrition and weights enigmas They also may have caused underhanded breath, left a bad taste in the child's jaws and resulted in throat pain that may have made swallowing difficult. As a inference a child may have become a finicky eater, developing more pronounced commons problems than those expected for his or her age. Gastrointestinal tip overs related to antibiotic therapy may have made family mealtimes challenging and unpleasant.

slumber problems. The child may be experiencing a certain degree of airway obstruction during be dead Even if he or she has not been diagnosed with steady sleep apnea, cor pulmonale, or alveolar hypertension, stertorous breathing may have disrupted the nap of the child and the entire, household. In fact, one parents arrive for a hospital appointment with a tape recording documenting their child's noisy repose The sleep-deprived child may be experiencing gymnasium or learning difficulties, enuresis, or behavior problems

Social question s A child may have missed day care, teach and parties. A special family trip or vacation may have been canceled or placed in jeopardy until the last minute. Parents may have raised work, educate and social obligations to care for their child at fireside take the child to the pediatrician's office, or wait for the physician to turn back their telephone calls.

for the use of all misperceptions. Another stress factor is that pediatricians, suckle practitioners, and physician assistants may have referr to T and A as a "last resort." a parents report that they were told children "grow into" their tonsils; thus, waiting to perform a manner of proceeding would be prudent. The parents may have been warned that T and A may not conclusion the child's throat infections. They also may have been told T and A is associated with the risk of hemorrhage or level death from anesthesia complications. Although these point to be solved or settleds occur, the parents may not have "heard" in the early discussions that they are rare. I have seen popular parent magazines describe T and A as an antique and dangerous surgery to be avoided at almost any sumptuousness and recent newspaper and television moderns reports also have warned parents of the dangers.
