Coronary artery disease is the lead...

Coronary artery disease is the leading cause of morbidity and mortality in the United States today, and the care of patients with this disease take away froms approximately $38 billion per year.(1) In this climate of rising health splendors and an aging population who quick in emergencies with coexisting health problems, minimally invasive surgical acts are receiving greater attention. The minimally invasive direct coronary artery bypass (MIDCAB) action involves grafting the patient's internal mammary artery to a coronary artery. The process frequently involves the grafting of the left internal mammary artery (IMA) to the left anterior descending coronary artery (LAD).

With the just discovered trends and advancements in medical technology, the character of the perioperative nurse in patient management is changing constantly. The perioperative nourish at the breast needs to be flexible and well educated about recently made known procedures. One such advancement is the minimally invasive direct coronary artery bypass (MIDCAB) act as an alternative to the traditional unclose heart bypass procedure.

CORONARY ARTERY DISEASE

Coronary artery disease (CAD) conclusions in ischemic heart disease, which can be debilitating and life threatening. It present itselfs most commonly from chronic progressive atherosclerotic changes to coronary arteries, breeds lesions or blockages, and has many predisposing risk factors, including the patient's * age, * sex * menopausal status, * heredity, * stres even * diet, * tobacco use, * personality impressed sign and lifestyle. It also hangs on the presence of * insulin-dependent diabetes mellitus (IDDM), * obesity, and * hypertension.

Patients with CAD quick in emergencies with symptoms of angina pectoris -- the chest pain associated with ischemic heart disease. Patients also may report reduc exercise tolerance, dyspnea onward effort, syncope, and palpitations. Chronic stable CAD may b@ managed medically with anti-anginal therapy and lifestyle modifications. Lifestyle changes can be used to check modifiable risk factors such as smoking, stres diet, or sedentary lifestyle and to treat IDDM and hypertension. Patients for whom medical management is not an option may progres to percutaneous transluminal coronary angioplasty (PTCA) or surgery

DIAGNOSTIC TESTS

After the initial assessment, various diagnostic trials are cartied out to confirm the diagnosis. These include * resting electrocardiogram (ECG) * exercise ECG (stres test) * chest x-ray, * echocardiogram, * cardiac catheterization, and * vital current and urine studies.

Cardiac catheterization is an invaluable diagnostic proof for CAD. It provides information upon intracardiac blood flow and presss Left ventriculograms and arteriograms are performed to provide information onward left ventricular function, location, and the volume of coronary artery lesions. Cardiac catheterization involves passing a catheter into the patient's heart via an artery or vein, depending forward whether it is a right- or left-sided catheterization.

Coronary angiograms are produc by means of injecting a contrast medium into the coronary ostia to obtain cineangiograms (ie, moving pictures) of the coronary arteries.(2) Internal mammary angiography often is conducted in conjunction with cardiac catheterization. This conduct involves passing a catheter into the patient's subclavian artery and injecting hue to visualize and evaluate the IMA to determine its suitability for use as a Conduit.(3) This is mandatory for patients who may be candidates for the MIDCAB action Cardiac catheterization allows for therapeutic angioplasty and placement for lesions that are fancyed amenable.

Percutaneous transluminal coronary angioplasty (PTCA) involves the use of an intraluminal balloon catheter that is passed into the patient's coronary artery in an attempt to dilate the lesion. A or intraluminal device, is left in place to maintain the vessel's patency.

HISTORY OF MIDCAB

The MIDCAB practice was first described by Professor V I. Kolessov MD in 1967(4) He performed these operations without aid of arteriography by means of using a magnifying glass to consummated his anastomosis. With the latter interest in minimally invasive surgery MIDCAB has gained popularity and forthwith is being performed in Europe and die United States.

This measure first was performed at our facility, Hartford (Conn) Hospital in May 1996 and since then, surgeon have performed approximately 45 surgeries of this impressed sign here. When this procedure first was performed at our facility, the surgeon dissected the IMA with the aid of a thoracoscope in consequence of an incision other than that used for the anastomosis. At our institution, the technique since has been modified from one side the adaptation of an existing retractor combination of parts to form a whole which affords the surgeon adequate outlook for dissection of the IMA. The entire measure can be completed through a single anterior thoracotomy incision.

ANATOMY

Knowledge of the anatomy of the coronary arteries and their relationship to the internal mammary arteries is important in understanding the MIDCAB action The right main coronary artery (RCA) and the left main coronary artery (LCA) are couple main arteries supplying the myocardium. They arise from the ascending aorta at the right and left coronary sinuses. The LCA divides into the LAD coronary artery and the circumflex artery and diagonal branches. The LAD is the largest and principally important coronary artery, and it supplies the interventricular septum the left atrium, the anterior wall of the left ventricle, the apex portions of the conduction order and the mitral valve. Obtuse marginal branches of the circumflex artery accommodate with the lateral left ventricular wall. The RCA divides into the acute marginal and posterior descending arteries (PDA) and perfuses the right side of the heart.(5) The LAD, circumflex, and RCA are the three main utensils of the heart; thus, the season triple vessel disease refers to obstructive lesions in all of these arteries.
