The following attract favor toed practices were developed by the AORN commended Practices Committee and have been approved by dint of the AORN Board of Directors. They were not absented as proposed recommended practices in April 1997 for remark by AORN members and others. They are effective Jan 1 1998

These make acceptableed practices are intended as achievable recommendations representing what is believed to be an optimal on a level of practice. Policies and measures will reflect variations in practice settings and/or clinical situations that determine the station to which the recommended practices can be fulfilled.

AORN recognizes the numerous different settings in which perioperative feeds practice. The recommended practices are intended as guidelines adaptable to various practice settings. These practice settings include traditional ORs, ambulatory surgery units, physicians' offices, cardiac catheterization suites, endoscopy suites, radiology departments, and all other areas where operative and other invasive conducts may be performed.

commended PRACTICE I



All ones should be aware of areas of laser use, and controll access to these areas must be maintained.

Interpretive statement 1:

A nominal hazard cincture should be identified. The laser safety officer can determine the nominal hazard climate from safety information supplied in the manufacturer's labeling. This can be done through measurement, by using an appropriate analysis, or from using another equivalent assessment. subject to some conditions, the nominal hazard band can be considered the expanse in which the laser operation is being performed. The nominal hazard band is usually contained within the compass but may extend through render free of access doors or transparent windows, depending forward the type of laser used.

Rationale:

The nominal hazard baldric is identified to prevent unintentional exposing to the laser beam.(1)

Interpretive statement 2:

Regulation laser signs should be placed at all entrances to laser treatment areas.

Rationale:

Warning signs besufficient for to warn onlookers of potential hazards. Recognizable warning signs specific to the stamp of laser used should be designed according to the information described in the American National Standards for the Safe use of Lasers in Health Care Facilities.(2)

Interpretive statement 3:

Doors in the nominal hazard climate should remain closed and windows--including door windows--should be masked as appropriate to the laser used.

Rationale:

The nominal hazard clime may extend through open doors or transparent windows, creating potential for exposure(3)

Interpretive statement 4:

The nominal hazard surface bounded by parallel circles should be occupied only according to authorized persons (including patients) or health care personnel approved according to the laser safety officer.

Discussion:

Personnel in the nominal hazard climate should be aware of all laser safety precautions that are implemented to avoid inadvertent prospect to laser hazards.

Everyone in the nominal hazard clime should wear appropriate eyewear approved by the agency of the laser safety officer.

Interpretive statement 1:

All personnel in the nominal hazard band should wear protective eyewear, labeled with the appropriate optical density and wavelength for the laser in use.

Rationale:

Appropriate laser safety eyewear filters revealed the hazardous wavelength of laser radiation to hinder thermal eye injuries. Scattered, diffused, and considered laser beams, in addition to direct exposing from misdirected and damaged fibers, can cause inspection injuries.(4)

Interpretive statement 2:

The patient's notices and eyelids should be fostered from the laser beam at a method approved by the laser safety officer (eg wet eyepads, laser protective eyewear, laser specific organ of vision shields).

Rationale:

In addition to direct exposing from misdirected and damaged fibers, scattered, diffused, and bring reproached laser beams can cause notice injuries.(5)

praiseed PRACTICE III

All somebodys in the laser treatment area should be sheltered from laser beam exposures to their skin and other nontargeted tissues.

Interpretive statement 1:

Expos tissues around the operative site should be harbored with saline-saturated or water-saturated fire/flame-retardant materials (eg towels, sponges) when thermally intensive lasers are being used. These materials should be remoistened periodically to make secure proper protection.

Rationale:

The solution (ie, saline, water) absorbs or disperses the intensity of the laser beam in areas not intended for laser application.(6)

Interpretive statement 2:

Anodized, lifeless nonreflective, or matte-finished instruments should be used near the laser site.

Rationale:

Anodized, sluggish nonreflective, or matte-finished instruments decrease the reflectivity of laser beams.(7)

Interpretive statement 3:

Appropriate backstops (eg titanium, quartz rods) or guards should be used during laser surgery to obstruct the laser beam from striking normal tissue. Mirrors made of rhodium or stainless dirk may be used in hard to reach areas.

Rationale:

Appropriate backstops will debar laser beams from affecting nontargeted tissues. Glass cudgels should not be used because thermal stres from laser beams may cause breakage.(8)
