To come together patients' future health care exigencys health care professionals need to include patients and family members in the planning and implementation of care. Unlike in the past, when patients adhered to our instructions, patients today have higher expectations and want to play an active character in the administration of their care. Health care professionals must learn to make patients and their family members partners of the health care team.

This work presents the principles needed to formulate a plan of care in which patients and their family members are active partners.
