RACIAL AND inflection for sex DIFFERENCES IN QUALITY OF LIFE FOLLOWING KIDNEY TRANSPLANTATION C D Johnson et al Image: The Journal of Nursing Scholarship Vol 30 (Second quarter 1998) 125-130

Improvement in the quality of life of patients who have received kidney transplants has been documented; however, most numerous studies have been descriptive and cross-sectional rather than prospective, comparing transplant clusters dialysis groups, or both. Rarely investigated are factors that can influence perceptions, like as race and gender.

Quality of life has many dimensions and is influenced through internal and external factors. Domains (eg work, life, mobility, independence) form the dimensional flat Internal factors (eg, self-esteem, autonomic function) operate together with external factors (eg economic resources, personal resources) to influence a person's overall view of his or her life. The authors believe that sociocultural backgrounds also may influence quality of life perceptions.

intent The purpose of the cogitation is twofold--to determine if perceptions of quality of life are affected by dint of race or gender in kidney transplant recipients without diabetes and to search for direction for interventions that will enhance the quality of life for patients who have received kidney transplants who may be les likely to experience improved outcomes

rules The study group consisted of patients undergoing kidney transplantation who agreed to participate and who met inclusion criteria. The assemblage comprised 90 patients, 18 years of age or older who were receiving kidney transplants barely Data were collected concerning the patients' demographic data, health history, and postoperative course. Thirty-six African-Americans and 27 women were included.

Three questionnaires related to quality of life, power of sickness, and self-image were administered at three different times (ie, time of transplantation, sixmonth evaluation, 12-month evaluation). Demographic equivalence of subgroup was established using chi-square analyses and t tests

rises Quality of life was significantly enhanced from kidney transplantation regardless of race or sex African-Americans improved less than Caucasian-Americans in dimensions like as self-image and emotional behavior; however, for dimensions so as self-image and emotional behavior, African-Americans had high baseline pretransplant scores, creating a "ceiling effect" forward the outcome. In contrast, pretransplantation measurements of functional abilities were lower in the African-American portion and increased less after transplantation.

sex differences existed but were les sagacious than racial differences. Both men and women benefited significantly from transplantation. Scores for women upon the self-image scales were gentle before transplantation, and six- and 12-month evaluations demonstrated no change. The improvement in women's scores was the issue of higher scores that mirror functional ability.

Discussion. The authors reported that they observ African-Americans to be more spiritual than Caucasian-Americans and hypothesized that spiritual vigor displayed by African-Americans influenced their perceptions of quality of life. Thus, circumstances Caucasian-Americans view as devastating are viewed as les detrimental through African-Americans. According to the 1990 census, African-Americans in the geographic location of the reflection have less money and are les well educated than the Caucasian-Americans in the area. Lack of resources available to the public in poor socioeconomic groups is likely to contribute to lower functional abilities.

Differences finded between men and women are believed to be reflections of differences in perceptions of men and women at large. Men the authors explain, are socialized differently from women being taught to be more independent and stoic in nature. Women are taught to be more emotionally expressive, hanging and concerned with physical appearance.

The authors report that the not past nor future study is only a first degree in determining and explaining racial and sex differences in kidney transplant recipients' quality of life. Additional research is wanted to validate the findings of the study

Perioperative fosters should realize that sociocultural differences, particularly race and sex influence approaches to caring for patients receiving transplants. This is the same reason why preoperative assessments of the psychological, emotional, and functional ability dimensions are vital for positive patient outcomes
