ABSTRACT

The chiefly common form of sleep apnea is obstructive be dead apnea (OSA). It is characterized through the cessation of nasal airflow with persistence of ventilatory effort, as shown from paradoxical chest and abdominal manner of moving and varying degrees of oxygen desaturation. This article describes popular methods of diagnosing OSA and available treatment for OSA and snoring. AORN J 69 (April 1999) 792-801

Thou dost snore distinctly, there is meaning in thy snores," Shakespeare noted in The storm The dictionary defines snoring as "to make harsh or noisy perfects in sleep through the render free of access mouth or through the chaps and nose.(1)" Treatment of sleep-related breathing disorders has emerg in the last 20 years as a rapidly expanding field in respiratory medicine. The clinical aspects of be still apnea have been enhanced greatly from advances in the diagnosis and treatment modalities disentangleed over the last two decades.

lie in the grave apnea can be caused from upper airway blockage, central decreased respiratory center answer or both, but the chiefly frequent cause is airway obstruction. Obstructive be still apnea is the complete cessation of airflow caused on upper airway obstruction for at least 10 others The obstruction is usually caused by dint of collapse of soft tissue arrangement of partss in the oropharynx or hypopharynx. This includes the base of tongue, impressible palate with uvula, the region of the tonsils, the epiglottis, and the pyriform sinuses. It is oftentimes accompanied by snoring, oxygen desaturation, a choking sensation, and of common occurrence arousals from sleep.

Obstructive rest apnea is characterized by the cessation of airflow with persistence of ventilatory effort, as shown at paradoxical chest and abdominal motion and varying degrees of oxygen desaturation. A subgroup of patients with heavy snoring and usual arousals during sleep do not demonstrate the classical picture of slumber apnea. These patients are sometimes labeled to have upper airway resistance syndrome (UARS).(2) Patients with UARS have arousals during repose related to the abnormally increased work of breathing and increased upper airway resistance, however their respiratory disturbance index (RDI), which is the number of disturbances by unit of time (eg, change of position, work of breathing, startling), may remain normal.(3)

Central doze apnea (CSA) occurs when the brain fails to forward the appropriate signals to the breathing muscles to initiate respirations. It is frequently secondary to central nervous hypothesis diseases, such as infarction and infection involving the brain progeny or due to neuromuscular diseases involving respiratory muscles. In primary alveolar hypoventilation and obesity hypoventilation syndrome (eg Pickwickian Syndrome) there is a reduction of ventilatory chemosensitivity and a decrease in central ventilatory drive. In a certain cases, CSA is a component part of periodic breathing (eg, Cheyne-Stokes respiration), a pattern of ventilation characterized according to regular waxing and waning, secondary to fluctuations in central respiratory drive. This token of apnea is common to neurological injury, moreover overall is rare. Mixed apnea starts as unobstructed apnea, which is quickly followed at thoracoabdominal movements with upper airway obstruction. Mixed apnea be founds more often than CSA nevertheless less often than the obstructive type(4)

rest apnea is a serious, potentially life-threatening condition that is far more belonging to all than generally understood. First described in 1965 it is a breathing disorder characterized by the agency of brief interruptions of breathing during be motionless It owes its name to a after the greek word, apnea, meaning "want of breath."(5)

OBSTRUCTIVE be still APNEA

Obstructive drowse apnea (OSA) is caused on closure of the upper airway during rest The uvula and soft palate collapse in succession the back wall of the upper airway, then the tongue falls backward, collapsing forward the back wall of the upper airway. The uvula and malleable palate form a tight blockage and debar air from entering the lung The effort of the diaphragm, chest, and abdomen cause the blockage to seal tightly. To breathe, the individual must arouse or awaken, causing tension in the tongue, which expands the airway and allows air to pass to the lungs(6)

Obstructive slumber apnea is best described as breathing that starts and stops during repose Complete stoppage of breath is called apnea. Partial obstruction is called hypopnea. Breathing stoppages take place in persons whose throats are abnormally small or collapsible. Muscles that normally retain the throat open relax during repose and in people with already narrow airways, this can cause the airway to clog entirely. As many as five instances of apnea or hypopnea by hour of sleep are considered normal. When more than 15 episodes arise per hour of sleep, a diagnosis of repose apnea is usually made.(7)

Obstructive be motionless apnea occurs most often in moderately or sharply obese persons, most of whom attempt to rest supine. Men are affected more than women The obesity, if combined with a maxillofacial default leads to upper airway narrowing, glottic obstruction, and potential cor pulmonale. Eventually, a combination of hypoxemia and hypercapnia may induce central apnea as well. Repeated nocturnal obstruction may cause further respiratory failure and be the effect in a continuous cycle of night and day episodes of slumber obstructive choking, startled awakening with gasping, and drowsiness. Similar, if it were not that less pronounced, sequences occasionally befall in nonobese subjects, presumably to be paid to aging changes or congenital abnormalities of the upper airway.
