When faced with question s such as ...

When faced with question s such as failing or improper equipment, perioperative supply with nourishments often are tempted to ask, "Why can't we commit to memory one that works?" These words begin a hectic scenario experienced in countles ORs. Potential point in disputes can range from malfunctioning instruments to scheduling disputes, late scope preparation, or lack of sufficient supplies or number of staff members. As in a play, the behind-the-scenes preparations, efforts, and interactions related to these riddles come to fruition--or frustration--intraoperatively. In answer to this, nurses are required to deliver immediate solutions and ofttimes do not have time to examine the issues objectively or collectively. The question s then can recirculate indefinitely as random corrective efforts ultimately fail.

The Japanese universal of "Kaizen" begins with recognizing that a moot point exists. Based on the universal if there are no question s there is no potential for improvement. Furthermore, a point to be solved or settled is defined as anything that subsequently inconveniences other the community (eg, people in the nearest process or the ultimate customers).(1) It is imperative, therefore, to recognize ongoing problems

The enigma tracking system offers a practical strategy for question at issue management in the new millennium. It is a standardized order that facilitates a continuous period of documentation, feedback, action, and assessment. The proces establishes an interdependence between question identification, solution, and evaluation, whereby all participants have a garmented interest in ensuring successful issues The process transcends staff members' disenchanted attitudes that oftentimes are reflected in comments as it was as "No one does anything anyway" or "It's just more paperwork." Instead, motivation is achieved or part of to the other recognition, reinforcement, and reward. Furthermore, through using the system,

* staff members become the principal participants in the process

* staff members are reassured via continuous feedback that their efforts are worthwhile,

* measurable succes is demonstrated, and

* staff members benefit by means of solving problems and making their piece of works easier.

The a whole components include the problem tracking record, the interrelated staff member affiliates, and the communication forms.

question at issue TRACKING RECORD

The question tracking record (PTR) is the driving force of the scheme as it provides the data to actuate the problem-solving proces (Figure 1) This "paperwork" is brief and simple and uses the expertise of those who have direct involvement with the question at issue Unlike formal reports that are limited to potentially injurious incidents, the PTR alerts staff members to communicate any moot point that impedes their work. The rule recognizes that, unless documented, the source and cumulative validity of a problem may remain unchecked.

[Figure 1 ILLUSTRATION OMITTED]

Origin of the enigma Investigating the origin of a question is similar to unraveling the scheme of a mystery. Many factors can contribute to a single incident, and the point to be solved or settled can recur if the origin of the vexed question is not accurately identified. Consider the following questions when investigating a problem

* Was the item or equipment strictly maintained, assembled correctly, and operated correctly?

* Were the staffing patterns and assignments consistent with the patient's needs?

* Was the course scheduled correctly?

* Was the point in dispute reported accurately and routed to the appropriate source?

* Was the report read, understood, and analyzed?

* Was the solution applicable, workable, and successful?

Based onward these types of questions, many variables of a riddle can be introduced.

Cumulative issue Unless a problem is tracked by dint of ongoing data collection, there can be no comprehensive analysis. For example, presume the wrong drill bit is uncloseed in the OR. This could have occurr for several reasons, so as the correct drill bit is in succession back order or is no longer used, the transaction is not scheduled to use a drill bit, or the correct drill bit has been mov to a different cabinet. The origin of the puzzle could be any or none of these. Furthermore, casting arbitrary solutions at presum question s can result in staff members' having a weary tolerance of the status quo More information is destitutioned to determine the reason for the point in dispute and its frequency. The PTR makes evident the "who, what, to what end when, and how" of an existing point to be solved or settled Accumulated PTRs help identify the greatest in number frequently occurring problems--the issues to prioritize for resolution--as they perish the greatest resources.

vexed question categories. The PTR provides a checklist to identify question s and to help categorize returning issues. For example, suppose a power drill did not function correctly during a process The drill malfunction would be classified as an equipment question The PTR would document the individual incident and, along with later PTRs, would be used to unravel measure, and assess a solution.

The PTR can be individualized to contemplate a specific work setting or to include question s frequently documented as "other." Generic categories like as communication or education are avoided, as these are inherent in in the greatest degree situations and, therefore, may dim the origin of the question An interpersonal classification (eg, conflicts about staff member responsibilities) also can be included. question tracking records related to interpersonal question s may be routed directly to a specific individual or placed in a separate collection chest to provide staff member confidentiality.
