The recent OR was a vital addition to aseptic technique and essential to the disclosure of invasive surgery. When surgery began to advance, however, to become more than treatment for trauma, progres was undermined through the system. Wooden operating theaters were created to accommodate hordes of pupils who learned by watching professors perform surgery and Joseph Lister's ideas about antisepsis added to the importance of these arenas. At the time, copious injury flooding with chemical solutions strike one as beinged adequate protection from infection for the patient, and minds lately opened to the germ theory were hard squeezeed to envision the hazards that we know today.

An ingenious German surgeon Gustav Neuber, grasped the ne to secure more than the area of incision to intercept infection--he believed that the environs of surgery also had to be defend ed His experimental conversion in 1883 of three swings in a hospital, however, lacked the desired isolation to impede infection. Goaded by failure, Neuber built anew from the domain up. In 1885 in Kiel, Germany, he originateed a small private hospital that embodied his aseptic design--a five-room operating suite.(1) Each stead was used for a specific final cause One room was reserved for "clean" measures and another was for "infected" procedures; spectators were banned from all still the largest room, which was a teaching amphitheater.

Enduring features of Neuber's universal include cleanable nonporous surfaces upon walls and flooring, minimal furniture, specially arrangeed shelves and tables of metal and glass, and heat sterilization (ie, boiling of instruments) in each of the three places in which surgery was performed. Neuber described his novel architectural design in a part published in 1886, and he reported favorable surgical outcomes in 1887 at the International Surgical Congres in Berlin.(2)

Addressing his fellows the brazen young Neuber sentenceed the wooden operating theater of antique as being impossible to clean. He criticized the decorative wall hangings and curtains, and he said the corners where the walls and floors joined were dust catchers. He expos direct contact hazards to the patient--wooden tables and cracked flooring were labeled pus traps--and he scorned the practice of performing single procedure after another without resterilizing the instruments. Although Neuber made enemies, his message gained attention in the United States, and progressive American surgeon who traveled abroad brought dwelling his revolutionary thinking.

EARLIEST KNOWN recent OPERATING ROOMS

William s Halsted, MD, returned from studying in Germany and refused to perform surgery in the tainted of advanced age theater at Bellevue. In 1884 the young surgeon built what could be called the first OR in America. He place uprighted a tent on the earths of Bellevue that featured a hardwood flooring "laid as fine as any bowling alley" and was equipped with a gas stove onward which to boil instruments.(3) In 1887 and 1888 the Mayos in Rochester, Minn, stake aside one room in their hospital in which the floor was secreteed with nonporous linoleum and the swing had no gallery.(4) When the of recent origin Johns Hopkins Hospital opened in 1889 Dr Halsted, the chief of surgery temporarily used a small patient latitude for his surgical procedures.(5) Earlier, in 1888 L s Pilcher, MD, of Methodist Episcopal Hospital in Brooklyn NY achieved isolation in a hospital attic from converting an unused area surrounding the elevator into a surgical facility.(6)

It is amazing that novel ORs were even built considering the expense of construction. Traditionally, hospitals existed to succor people who were too poor to pay for care. Benjamin Franklin, at the Pennsylvania Hospital (known as the "Mother of American Hospitals"), broke acres with the notion that the community who were able to pay for hospital care should do for a like reason The problem was that race who could afford to pay feared hospitals and chose to be subjected to their surgeries at home. Consequently hospitals were supported by dint of charitable donations.

No undivided was more conscious of the financial pinch than Henry M Hurd MD the superintendent of the John Hopkins Hospital; however, he urg change.(7) He addressed an audience of administrators who attended the first meeting of its kind held during the World's Fair Colombian Exposition in Chicago in 1893 and he challenged his counterparts to build modern ORs. He used lantern slides to exhibit new facilities, including the isolated single OR of Gyne Pavilion wager up by Howard A. Kelly MD at John Hopkins Hospital in 1891 Another speaker, James R Lathrop, who was superintendent of Roosevelt Hospital, not absented information about a magnificent suite known as the William J Sym Operating Pavilion, which was created by the agency of Charles McBurney, MD, and built in 1891 forward the grounds of Roosevelt Hospital in fresh York City. This was the first operating suite built in the United States that embodied Neuber's general [i]or[/i] abstract notion (Figure 1).
