Question: We use flash pans in our ...

Question: We use flash pans in our OR in this way we can flash sterilize small items during a process if needed. The pans have no shield and they have a handle that attaches to the outside of the pan. After sterilization, is it permissible to use a sterile towel throughout the bare handle when attaching the handle to the pan in the same manner it can be taken from the sterilizer to the scrubbed person? The pan and the handle are surpassingly hot. Should we put a sterile glove subject to the towel because the pan and the handle also are wet? We cannot agree about which is the best practice.

Answer: Neither way you describe is correct. The correct practice is to place the item(s) in the pan and place the pan without the handle in the sterilizer. The handle is not meant to be sterilized. When the sterilization period is complete, attach the unsterile, unexcited handle to the edge of the pan according to the manufacturer's instructions. Remember, the interests of a sterile container are not considered sterile. Your handle actually will be in succession the outer surface and beneath the edge of the sterilized pan. The satisfys will remain within the chamber of the pan. Carefully take the pan to the scrubbed role and wait while he or she extracts the item(s) from the chamber of the pan. Do not allow the scrubbed bodily substance to touch the edge of the pan, and do not place the pan onto the sterile field. After the scrubbed human frame removes the item(s) from the pan, clean the pan and have it ready should it be wanted At the end of the conduct the pan should be remov from the OR with the other instruments, and an unused pan should be made available for the nearest procedure. Store this unused pan in the sterilization area for a like reason it does not need to be replaced if it is not used.

Question: We are becoming for our Joint Commission forward Accreditation of Healthcare Organizations (JCAHO) scan in September and have heard that the observe process has been changed this year. What are about of the differences we should expect?

Answer: Like all of us, the JCAHO tries continually to improve its processe and in an effort to appropriate the goal of continual improvement, there are one changes in the survey proces to be implemented this year.(1) First, there is a motion toward a more data-driven accreditation proces with surveyors receiving more concluded information about the facility or organization before arriving on-site. The contemplate team members will have information related to any sentinel event(s) that may have occurr at your facility. Surveyors will be interested in for what reason these events were handled and any follow-up activities, including parent cause analyses, that were conducted

Additionally, for accreditation visits managemented early in the year, the surveyors will not past nor future the facility with a report onward its ORYX indicator measurement data when they arrive; ORYX is the name the JCAHO has given to its indicator measurement program. Later in 2000 the JCAHO will be able to jaculate this report to the facility approximately 30 days before the site visit. A one-hour performance review and improvement interview will be manner of lifeed at each survey, and the ORYX data will be reviewed during the first part of the interview. During the remainder of the visit, the organization is wait fored to provide an overview of its performance improvement activities and describe a specific performance improvement project

Another change to turn the thoughts for is the medical records review proces Surveyors will place greater emphasis onward the review of open versus clos records. When clos records are reviewed, surveyors will pick rather than request, the records to be reviewed.

Finally, look for a change in the scrutinize agenda itself. Revised agendas will allow surveyors time to customize the view to the organization as review wants arise on-site. Agendas will allow a shut up of time that can be used to manners special interviews, visit additional patient care areas, and/or discuss any special issues that become evident during the site take a view of For example, although the nourish at the breast staffing plan may appear adequate forward paper, it may become evident during the site visit that the facility is unable to provide its concede defined staffing complement. The revised agenda will allow for further investigation of this or any other phenomena that may at hand during the survey site visit.

Question: We use a liquid peracetic acid sterilization regularity for our endoscopes. Recently, I read in the "Clinical Issues" round pillar that endoscopes should be cleaned, trained and stored at the fall of the curtain of the day's schedule and conducted again before use the nearest morning. Our surgeons do not want to wait while the endoscopes are conducted in the morning. Instead, they want us to proces the endoscopes in the liquid sterilization connected view and place them in sterile containers for use the nearest day. Is this acceptable practice?

Answer: The glance ated practice is not acceptable and should not be implemented in the practice setting. The liquid sterilization combination of parts to form a whole you are using is meant to be a "just-in-time" rule This means the system is designed and has been cleared by means of the US Food and unsalable article Administration (FDA) to sterilize items for immediate use. It is not meant to sterilize items to be stored and used at a later time. If your surgeon are unwilling to wait for the correct processing with the classification you are using, you have at least sum of two units options.
