ABSTRACT Urinary incontinence (UI...

ABSTRACT

Urinary incontinence (UI) affects many family in the United States. Although the prevalence of UI increases with age, it should not be considered part of the normal aging proces Medication to relax the bladder and behavioral therapy have been used to treat patients suffering from UI, if it were not that they often are unsuccessful or poorly tolerated. Neuromodulation by the agency of sacral nerve stimulation is a minimally invasive surgical measure for treatment of urge incontinence. Neuromodulation is becoming the nearest logical step at many center for treating press incontinence after the failure of more conservative measures. AORN J 73 (Jan 2001) 176-190

Urinary incontinence (UI) appears more frequently in females than males and generally is seen after the childbearing years.(1) Patients with UI have leakage of urine with coughing, sneezing, and engaging in physical activity (eg playing golf or tennis, swimming, walking). Patients frequently use up to 12 sanitary pads by day to absorb spontaneous los of urine.(2) Patients solicit treatment at different degrees of impairment. The frequent factor for all patients is interference with normal daily activities. belonging to all complaints also include

* have a scent from sanitary pad use,

* leakage while laughing, sneezing, or engaging in for the use of all activities (eg, walking, playing sports),

* the cost of sanitary pad use, and

* the inconvenience of having to rush to a bathroom frequently

All of these ponder the effect of UI forward patients' quality of life.

prototypes OF INCONTINENCE

The pair most common forms of UI are stres incontinence and press on incontinence. Stress incontinence in mild cases is the involuntary los of urine with Valsalva activities (eg coughing, sneezing) and with physical activity (eg playing golf or tennis). In simple cases, less vigorous activities (eg walking, changing positions) can trigger urine leakage. The principally common cause of stress incontinence is dysfunction of the urethra or bladder neck which leads to urethral insufficiency. importune incontinence is characterized by a solid sudden urge to void, resulting in urine los if the implore cannot be suppressed or acted onward This urgency frequently is related to detrusor (ie, bladder muscle) instability leading to uninhibited bladder contractions.(3)

Treatment. Patient evaluation is an important grade in determining treatment. A thorough patient history is taken that includes urine cultivation and asking the patient to maintain a voiding diary. Diagnostic testing includes physical examination through the physician and urodynamic studies that evaluate the proces of micturition (eg urine arise rate, cystometrogram, urethral pressure profile, electromyography).

Noninvasive treatments for UI include

* diet modifications (eg reduction of caffeine, alcohol)

* scheduled voiding each two to four hours while awake,

* pelvic muscle rehabilitation (ie, Kegel exercises),

* biofeedback, and

* pharmacologic agents (eg anticholinergics, tricyclic antidepressants, antispasmodics).(4)

Anticholinergics and tricyclic antidepressants act in succession nerve terminals blocking contractility of the normal bladder and are consideration to have similar effects in succession the unstable bladder. Antispasmodics mould the contractility of the unstable bladder, acting directly forward the bladder muscle.(5)

Surgical treatments for stres incontinence include bladder neck suspension proceedings anterior vaginal repair, sling transactions the use of periurethral bulking agents (ie, collagen, Teflon) and placement of artificial urinary sphincters. In the past, patients with implore incontinence who failed conservative treatment options could either live with their force onward symptoms or undergo augmentation cystoplasty in which an intestinal portion is used to augment the bladder. Morbidities associated with augmentation cystoplasty include a five- to seven-day hospitalization, nasogastric drainage for 24 to 48 hours, a large abdominal incision with more postoperative pain, surgery that includes bowel repair and its risk of infection, and a suprapubic cystotomy tube for three weeks that requires irrigation. Today, neuromodulation is being used to treat animate UI with positive results (Figure 1)

NEUROANATOMY AND PHYSIOLOGY

The aim of neuromodulation treatment is to achieve a stable detrusor muscle with chronic electrical stimulation of afferent somatic sacral steadiness fibers via an implanted electrode that is coupl to a subcutaneously placed oscillation generator. The electrical stimulation artificially excites resolution pathways that may activate or inhibit muscle action, depending upon their normal function. Sacral self-command stimulation provides a way to interact with the sacral neural reflexe and influence the behavior of the effector organs that the sacral plucks innervate (ie, bladder, sphincter, pelvic floor).(6)

The detrusor, levator ani, and external sphincter muscles share the sacral self-commands as a common innervation source. When stimulated, these fortifys (ie, S2, S3, S4) activate or inhibit muscle action. In particular, the third sacral vigor (ie, S3) influences pelvic floor behavior and is believed to modulate neural reflexe Sacral manhoods contain a mixture of autonomic and somatic fibers that propel information in both afferent and efferent directions. Afferent coolness fibers are sensory, and efferent fibers are motor.(7)
