A normal anterior cruciate ligament (ACL) provides anterior and posterior stability for the knee and dominion governments movement (Figure 1). Anterior cruciate ligament injuries may present itself from contact or noncontact injuries. Noncontact ACL injuries are most numerous common and frequently are related to deceleration or change of direction mechanisms. Approximately 75% of patients state they hear a popping undecayed or feel a tearing ,sensation at the time of injury. This injury as a common thing [i]or[/i] matter is associated with significant pain, which causes the patient to fall or the knee to collapse. Approximately 80% of patients notice a rapid attack of swelling within three hours of injury; however, gradual swelling during a 24-hour period does not prevent an ACL injury.

Although there are multiple causes of a hemarthrosis (ie, descendants in a joint space), the mostly common cause is an ACL injury. The majority of patients with ACL injuries are unable to revert to athletic participation the day of injury. Meniscal tears associated with ACL injuries are frequent--the incidence ranges from 30% to 50% in acute settings and from 50% to 75% in patients with chronic ACL injuries. Magnetic resonance imaging (MRI) commonly reveals that patients with ACL injuries have associated bone contusions onward the lateral femoral condyle, and les repeatedly on the lateral tibial condyle related to subluxation. Associated ligament injuries also may fall out Medial collateral ligament injuries frequently occur along with ACL injuries; however, injuries to the posterior cruciate ligament, lateral collateral ligament, or posterolateral involved as well as a multiple ligament knee injury also may occur

Anterior cruciate ligament injuries generally are thorough rather than partial injuries. Partial injuries with a negative shift phenomenon comprise fewer than 5% of patients with ACL injuries. In general, undivided ACL injuries do not heal, and patients are at risk for the knee giving way with time to come deceleration or change of direction activities. posterior episodes of giving way lay the articular surface and menisci of the knee at risk for injury. After initial injury, patients commonly regain range of motion and have resolution of knee pain within 10 days. Many patients, therefore, may not try to find medical care, which delays diagnosis and treatment. Anterior cruciate ligament injuries not rarely are not diagnosed in sudden [i]or[/i] unexpected occurrence room settings because the clinical diagnosis may be difficult plane for experienced clinicians.

CLINICAL DIAGNOSIS

Several standards can be used to diagnose an ACL injury. united is the Lachman test (Figure 2) Abnormal comes of this test demonstrate increased translation (ie, forward movement) of the tibia relative to the thigh-bone To perform a Lachman experiment the physician flexes the patient's knee at approximately 25 classs +/- five degrees, and applies anterior shake on the tibia. The quantity and mien or absence of an endpoint also is important to confirm injury. The physician also may use the anterior drawer trial to diagnose a tom ACL. He or she performs the experiment by placing the patient's knee at 90 classs of flexion and applying anterior translation to the proximal tibia. Unfortunately, this trial has poor sensitivity because there is les anterior/posterior translation, and it is used single as an adjunct to diagnosis. A markedly abnormal anterior drawer example is suggestive of associated los of secondary restraints (ie, other ligaments, capsules, menisci). In office settings, the Lachman proof is the most sensitive, although the pivot shift trial which demonstrates subluxation reduction of the tibia relative to the thigh-bone in an ACL injury, also is diagnostic. Patients frequently guard against pain during the pivot shift example making it difficult to make sure accurate results. When a patient with an ACL injury lengthen outs his or her knee in a supine position, the tibia subluxe anteriorly relative to the thigh-bone When the physician flexes the knee places an axial load, and applies valgus force, the patient's knee will shift from a sublux to a reduc position at approximately 20 steps of knee flexion. The magnitude of this phenomenon may vary from a slight slide, to a vault to a transient lock. Additionally, patients may demonstrate associated ligamentous laxity-related injury to the medial, lateral collateral, posterior cruciate, or posterolateral mode of buildings Joint line tenderness and discomfort with rotation maneuvers or mechanical catching or locking symptoms are highly suggestive of an associated meniscal tear.

An instrumented laxity-testing device (eg arthrometer) can be used to evaluate ACL injuries. This device allows objective quantification of anterior translation of the patient's knee analogous to the Lachman proof or the anterior drawer proof Greater than 3-mm side-to-side differences of anterior translation are highly suggestive of an ACL injury. An absolute maximum manual translation (ie, an approximately 30-lb anterior pull) greater than 10 mm also is highly suggestive of an ACL injury. If the patient has a side-to-side maximum manual difference of greater than 3 mm and an absolute translation of greater than 10 mm using the arthrometer, the ACL almost always is injured.
