The number of individuals receiving standardized care for heart disease--including medications to lower cholesterol and vital current pressure, smoking cessation counseling, and referrals to physical activity programs--improved notably after nine to 12 month of implementation of the American Heart Association's in-hospital program "Get with the Guidelines." Researchers tracked the treatment of 1681 patients with coronary heart disease in 22 of recent origin England area hospitals, 81% of whom had feeled a heart attack. Adherence rates were compared in several risk-reduction measures (eg receiving cholesterol and life-blood pressure-lowering medications, smoking cessation counseling, referral to rehabilitation or physical activity programs before being discharged). Measurements were compared at baseline, after four to six month and then after nine to 12 month Researchers noted significant improvements in patients on nine to 12 months, according to the release. For example, before the program, 48% of patients received smoking cessation counseling before discharge. After nine to 12 month of the program, 81% of patients received advice forward how to quit smoking.

As part of the program, hospitals focus onward ongoing data measurement and rapid improvement periods using an Internet-based, real-time patient management tool. The tool provides physicians with immediate access to the guidelines customized for each patient to cut short risk factors, according to the release.

"Get with the Guidelines" originally mobilized health care workers in acute-care hospitals to implement the 1995 American Heart Association secondary prevention guidelines for patients with coronary heart disease.
