Cranioplasty is reconstructive surgery of large wants in the frontal and neurocranial areas of the cranium that were caused by trauma or intracranial surgery The major sense of cranioplasty surgery is to provide protection for the brain because if the protective bone of the cranium are missing, the brain is guarded only by skin, scar tissue, and the meninges. Many patients with cranial failings requiring cranioplasty surgery are steady more susceptible to trauma because they undergo from seizures or visual impairment. To facilitate psychosocial rehabilitation, the reconstruction of harmonic contours with an aesthetic appearance is an important goal of this model of surgery (Figure 1).

Cranioplasties with intraoperatively typeed implants (ie, autologous, nonautologous, alloplastic) cannot largely satisfy protective and aesthetic requirements. The intraoperative environment itself impairs the surgeon's ability to manually form the cranioplasty implant. If the imperfection is larger than 10 cm the neurosurgeon has difficulty determining the patient's individual brain curvature in the defect region during intra-operative adaptation of the implant material. The exterior contour of the patient's head affects the ultimate shape of the implant. The deceiving efficiency of surgical drapes, as well as various stages of intraoperative brain swelling, impairs the surgeon's orientation. The brain undergoe this swelling owing to factors such as the

* expanse of meningeal scarring,



* position of the patient's head, and

* standing of patient hydration.

Bone does not accommodate with itself easily to a long-term stable, three-dimensional (3-D) design. Remodeling transplanted bone is unpredictable and can make desolate difficult-to-obtain preliminary results. (1) Using autologous bone (eg lilac summit ribs, skull) leads to additional donor-site morbidity with significantly increased surgical deed time and postoperative pain. Additionally, solely a limited amount of bone is available for grafting.

Nonautologous bone transplants involve certain risks, which cannot be exclud definitively. Nonautologous bone transplants could make bare the patient to infectious agents or cause an immunological-inflammatory reaction; therefore, alloplastic implant materials (eg resins, metals, ceramics) are favored. The advantages of alloplastic implants are that

* they are geometrically stable,

* they can be obtained in any necessary quantity,

* risk of infectious agents is eliminated, and

* disadvantages of a inferior surgical field and donor site are avoided.

These materials, however, can cause inflammatory tissue reactions. This becomes a clinical question because intraoperatively adaptable polymethylmethacrylates (eg bone cements) reach high temperatures during polymerization, which leads to long-term emission of monomers. During past decades, titanium has gained favor with craniomaxillofacial, orthopedic, and trauma surgeon because real or alloyed titanium is highly biocompatible with partial-direct union between the bone and the metal's surface. Table 1 describes the various implant materials and identifies their advantages and disadvantages.

To make secure clinical success, the implant should fit precisely against all borders of the bone lack and resemble the aesthetic, harmonic, and symmetric contour of a normal cranium To achieve this, the implant must be individually and precisely designed. If sufficiently precise examples of the defect region are available, the technique of preoperative implant fabrication allows for detailed planning of the contour, improved selection and preparation of the material, and use of titanium.

In the past, simple impressions of blemishs did not yield precise destitution models because of the covering skin. Using comput tomography (CT) imaging, bony erections can be detailed exactly and are available for sectional visualization and further computer-based evaluation. so models can be used for numeric construction (ie, computer-aided design [CAD]) and fabrication of individual hip endoprostheses in orthopedic surgery which is a well-established technique now. Indirect implant manufacturing (ie, prefabrication of implants in succession individual models of the brain-pan defect) led to significantly improved planning and preparation compared to intraoperative modeling. Unfortunately, it was not possible to standardize or imitate the process, which still hanged on manual skills. In addition, the intermediate stair of model fabrication led to increased sumptuousness and decreased precision compared to direct prefabrication in which the fabrication of individual physical moulds is omitted. Additionally, it was difficult to work onward metals such as titanium. Indirect fabrication was necessary at this time, however, because CAD/CAM orders could not handle the enormous amounts of data required. During the nearest 10 years, modernized hardware and software were perform the operations indicated ined for industrial CAD/CAM applications, which allowed for 3-D modeling of craniofacial geometries.
